The active-tooth diagnoses card view-model — mirrors getActiveRootPerio's
active-tooth guard shape. rows covers the RAW derived set (each tagged
suppressed per the tooth's dxOverrides) plus any add-mode override key
that no rule derived; addableKeys is every other tooth-level diagnosis key
not already covered by a row, for an "add a diagnosis" picker. Localized
labels are resolved by the CARD via t("dx."+key) — this returns raw keys +
codes only. visible is false (empty rows/addableKeys) with no active tooth
or on a tooth that isn't naturally present (missing/implant/under-gum/
extraction-socket — the same gate deriveDentalDiagnoses itself applies).
