Rebuild the SVG tooth grid after the chart column remounts (composable-UI
Tier 2). Preserves the current selection/active tooth across buildGrid()'s
internal reset, clears+rebuilds the four grid element maps, and bumps
initToken so any in-flight/toggle-spam build supersedes cleanly. No-op
before init or if a newer build/teardown supersedes this one mid-await.
