apicalDx values (abscess forms, condensing osteitis, normal) carry no
granuloma/cyst refinement. Non-present teeth keep apicalDx="normal" (their
glyph is driven by mods.inflammation in the render, unchanged) so the row is
hidden for them — subtype authoring on implant/missing is deferred to the
peri-implantitis sub-project.
